Roles and responsibilities of this job

We are seeking a qualified, motivated, and patient-focused Clinical Pharmacist to join our healthcare team. The successful candidate will be responsible for providing clinical pharmacy services, optimizing medication therapy, promoting rational and safe use of medicines, and working collaboratively with doctors, nurses, and other healthcare professionals to improve patient outcomes.

Key Responsibilities

Review and evaluate prescriptions for appropriate drug selection, dosage, route, frequency, and duration.
Monitor patients' medication therapy and identify potential drug-related problems.
Check for drug–drug interactions, contraindications, allergies, and adverse drug reactions.
Participate in clinical/ward rounds and provide pharmaceutical care recommendations.
Provide medication counselling and education to patients and caregivers.
Provide evidence-based drug information to doctors, nurses, and other healthcare professionals.
Monitor, document, and report adverse drug reactions and medication-related incidents.
Promote rational and safe use of medicines and support antimicrobial stewardship activities.
Conduct medication reconciliation during patient admission, transfer, and discharge.
Support prescription audits and medication-use evaluations.
Assist in developing and updating hospital formularies, treatment guidelines, and pharmacy SOPs.
Maintain accurate clinical pharmacy records and documentation.
Participate in medication safety and quality-improvement initiatives.
Ensure appropriate storage, handling, and management of medicines.
Support inventory monitoring, including near-expiry and expired medicines, when required.
Participate in clinical research, audits, training, and professional development activities.
Work closely with multidisciplinary healthcare teams to ensure safe and effective patient care.
Perform other pharmacy-related responsibilities assigned by the supervisor or hospital management.

Required Qualifications

Master's Degree in Pharmacy (M.Pharm), preferably in Clinical Pharmacy, Pharmaceutical Care, or a related specialization.
M.Pharm or equivalent qualification from a recognized institution.
Valid registration with the Nepal Pharmacy Council (NPC) or eligibility for registration.
Minimum 2 years of experience in hospital/clinical pharmacy is preferred.
Candidates with relevant postgraduate clinical pharmacy training and strong academic backgrounds may also be considered.

Required Skills & Competencies

Strong knowledge of pharmacotherapy and clinical pharmacy practice.
Good understanding of drug interactions, adverse drug reactions, and medication safety.
Excellent communication and patient-counselling skills.
Strong analytical, clinical decision-making, and problem-solving abilities.
Ability to work effectively in a multidisciplinary healthcare team.
Good documentation and computer skills.
Strong attention to detail and commitment to patient safety.
Ability to work in a fast-paced hospital environment.
Professional, ethical, and patient-centered approach.
