Request for Quotation (RFQ) for Medical Equipment, Test Kits, Reagents and PSA Oxygen Plant

FHI 360, an international NGO, requests for quotation from locally registered agencies to supply the following items:

Technical Specifications’ Form (TSF) with detailed technical specifications of the above-mentioned items can be obtained via email request to [email protected] The following documents should be submitted along with the quotation:

1. A copy of PAN/VAT registration certificate
2. A copy of Company registration certificate
3. Latest Tax clearance certificate
4. Manufacturer’s Authorization
5. Company Profile
6. List of Clients
7. Duly filled TSF and documents requested in the TSF

The quotation should be valid for at least 90 days and must provide information on delivery time, warranty period & after sales service (for applicable items) and product shelf-life (for items containing expiry dates). Final selection will be based on favorable cost & services, delivery time/capability and fulfillment of minimum required product shelf-life for items containing expiry dates. Preference will be given to agencies who have the required items in stock or will take less delivery time.

Hard copy of the quotation along with specified supporting documents requested can be submitted in a sealed envelope clearly marked “Quotation for Medical Equipment, Test Kits, Reagents and PSA Oxygen Plant” at the following address:

Deadline for submission of quotation is June 21, 2021, 5:00 p.m. Quotation received after the deadline will not be considered for evaluation.
