Aligned Community Care Pty Ltd –Disability Support Company that provides service in Australia. We are in the course of establishing an offshore office in Nepal to handle client case management.

We have a current vacancy of 1 Client Case Manager (CCM).

Each Client Case Management Officer looks after a caseload of 10 to 15 disability clients.
The Client Case Manager supervises CCMOs, ensures the duties are completed regularly, and reports to the senior management team.
